Advanced pEEG Symposium

Due to overwhelming popularity the format for the Advanced EEG session has changed to accommodate more delegates. Rather than a small group workshop, the session is lecture format with ample opportunity for discussion, questions and the usual top quality brain banter. We look forward to, and encourage interaction from all participants.

 

Covering: Revision of density spectral array and drug effects. The EEG in the aging and developing brains. Burst suppression and brain vulnerability. EEG sensor placement and montages. Pro:Con of all pEEG systems. Recommendations for set up and interpretation. Clinical cases will be used throughout. It is anticipated that participants routinely use pEEG in their practice and are familiar with the basic terminology and pattern recognition of raw and processed EEG parameters.​

The intention of the workshop is to cover the relevant electrophysiology of the brain, how the drugs we give as anaesthetists (including volatiles!) alter the EEG and how the evolution of pEEG is allowing anaesthetists to increase their confidence in delivering safe and personalised anaesthesia to their patients. The workshop will go "behind the number" looking at the raw EEG trace, power spectral analysis and density spectral array.

This workshop will cover the basic aspects of TIVA for children and allow participants an opportunity to discuss this technique with expert paediatric TIVA users.

There are a variety of drugs other than propofol and remifentanil that anaesthetists can incorporate in a TIVA technique. Examples include ketamine, alfentanil, fentanyl, etomidate and dexmedetomidine. During this workshop we will describe and discuss indications, and use cases, and touch on the pharmacokinetics and dyamics of these drugs. The advantages and disadvantages of TCI administration of these drugs, and of the available PK/PD models will be explained. The knowledge gained will help to inform safer and better administration of these drugs, whether by TCI or manually controlled infusion.  For delegates wishing to administer these drugs using a commercially available TCI pump programmed to administer one or more of these drugs, we will describe the model development process, and the limitations and disadvantages of the relevant models. For delegates wanting to administer one or more of these drugs by manual information, a better understanding of the pharmacokinetics of the drugs, along with simulations using relevant models, will help inform safer, more rational manual administration.

Delivered by two experienced TIVA users with infectious enthusiasm, this interactive workshop is designed for senior anaesthetists looking to cultivate the next generation of confident intravenous anaesthetists. It is suitable for consultants, SAS doctors and senior resident doctors. In this workshop, we hope to deliver a blueprint for demystifying TIVA in clinical education, and introduce beginners safely to target-controlled infusions (TCI), troubleshooting common technical anxieties, and embedding safe practice guidelines into daily use. The workshop will help attendees translate complex pharmacokinetic and dynamic principles into scientific understanding tailored to the FRCA syllabus requirements. Participants will hopefully leave with the required strategies to champion safe, confident, and syllabus-aligned TIVA practice within their own departments."

TIVA in Special Circumstances

Workshop Tutors: Dr Tim Knowles & Dr Yavor Metodiev

The intention of the workshop is to explore how total intravenous anaesthesia can be tailored to patient groups where the usual approach demands rethinking. We will cover the pharmacokinetic and practical considerations of TIVA in obstetrics, including placental transfer and the shift towards propofol-based techniques for caesarean delivery; the use of TIVA for rapid sequence induction, with clinical tips and discussion of the recent controversy around its role; and dosing in obesity, where the choice of covariate and pharmacokinetic model has a direct impact on titration and recovery. The workshop will take a practically orientated approach to these challenging scenarios.
